At Careology, we're always working closely with our partners to ensure the platform reflects their needs. One of the most frequently requested enhancements was clearer documentation of what actions have been taken during patient interactions — and the outcomes of those actions.
With that in mind, we’re excited to launch our new and improved Action & Outcome Log, a new enhancement to a current feature built to support clinical collaboration and generate meaningful data to help understand the impact of clinical interventions.
Our original “Actions Log” was a useful tool — but over time, we heard that it led to inconsistent data entry, making it harder for teams to interpret what actions had been taken, and the associated outcomes.
The redesigned feature provides clear separation between:
Action – To confirm what the interaction with the patient was (e.g. “telephone assessment”, “in-person assessment”)
Outcome – To capture the result or recommendation from the interaction (e.g., “advice given, supported self-management”, “referred to a specialist”, “advised to go to A&E”)
Note – To optionally provide additional relevant context
This structured data is still visible on the patient card and patient details page, supporting joined-up working when multiple professionals are involved in care.
This tool is not a replacement for formal documentation in your EHR. It is an adjunct tool, helping your team work together more effectively and capture the value of interventions across the cancer pathway.
When viewing a patient, Click ‘log action’
Select the action you've taken from the drop down
Select the outcome
Write any notes to give further context
Click “Log” to save the information provided and keep a record of the patient interaction

If you undertake an action with a patient, be it on a call or in a clinic, you can log the action taken, the outcome and any other notes. This can be anything from a change of medication, to a referral or to general advice. The information recorded on Careology is displayed to all professionals with the same access to the patient record on Careology. Please note, patients will not see what is recorded.
Using the improved Action & Outcome Log creates visibility of which professional user has intervened, when and how, for each patient in a systematic way. This allows clinical teams to work effectively, by avoiding duplicate interactions with the same patient on the same matter and creating an environment for more proactive, personal and better-informed cancer care.
